- Resident Card / Work Authorization Card (if applicable)
- Auto Insurance (if driving for clients)
- CPR Card (available onsite if you don’t have already – $50 cash / $55 card)
- Voided Check or Direct Deposit Form
- DOEA 1 hour ADRD Certificate
- DOEA 2 hour ADRD Certificate (Coming soon)
- Level 2 Background Check (only needed if you have not done the screening within 4 years) Onsite available in Venice $90 cash or $95 Credit card.
